Arterial Treatments

PERIPHERAL ARTERY DISEASE (PAD) PAD develops most commonly as a result of atherosclerosis, or hardening of the arteries, which occurs when cholesterol and scar tissue build up, forming a substance called plague inside the arteries resulting in narrowing or complete blockage of the artery. 


Symptoms 

  • Leg pain, cramping, heaviness or weakness 
  • Leg numbness or weakness 
  • Sores on the legs, feet or toes 
  • Change in skin color on the legs.

  3. Atherectomy

  • Rotational Atheractomy


 

Laser Atherectomy:


A small  Laser catheter attached to a Laser machine is advanced either through  the groin artery OR artery at the level of the wrist over a thin guide  wire. The Laser catheter is inserted into the artery with blockage after  confirming the location of the catheter under x-rays, the laser machine  is activated, laser energy emitted at the catheter tip evaporates the  buildup inside the artery opening the blocked artery and also modifies  the vessel wall, so that it responds better to the balloon.

Sedation: Intravenous Sleeping medication(midazolam), as well as pain medication (fentanyl) is given.

Post  procedure instructions: Sometimes a stitch or plug is used to close the  small entry puncture site in the artery in the groin.

Patient  usually has to take bedrest for 2 to 3 hours at the surgical center.  After that patient will be discharged. At home patient is usually  advised to relax and rest with limited walking to the bedroom and  bathroom as necessary for 24 hours avoiding any kind of weight lifting  or exercise for 2 days. Normal activities are allowed after 2 days.

If  the entry site into the artery is at the level of the wrist these  patients can go home early and there are no restrictions with walking or  exercise following the procedure after 24 hours .

Directional Atherectomy:



A  small specially designed catheter which can cut the plaque ( the build  up causing the blockage from inside the artery) is advanced either  through the groin artery OR artery at the level of the wrist over a thin  guide wire which is inserted into the artery with the blockage. After  confirming the location of the catheter under x-rays, the catheter is  activated which cuts the buildup (called plaque) inside the artery which  is collected in a chamber at the tip of the catheter which is brought  out of the artery, the chamber is cleaned after taking it out of the  artery and reinserted if additional treatment is needed.


Sedation: Intravenous Sleeping medication(midazolam), as well as pain medication (fentanyl) is given.
Post  procedure instructions: Sometimes a stitch or plug is used to close the  small entry puncture site in the artery in the groin.

Rotational Atherectomy :


A  small specially designed catheter which can cut and aspiration the  plaque ( material causing the blockage from inside the artery) is  advanced either through the groin artery OR artery at the level of the  wrist over a thin guide wire which is inserted into the artery with  blockage after confirming the location of the catheter under x-rays, the  catheter is activated which cuts and aspirates the buildup inside the  artery.
